Post-treatment stability is one of the most important and desirable goals of orthodontic treatment. This lecture will critically review the changes that occur during and after with various forms of Class II treatment. Emphasis will be placed on distinguishing the treatment and growth changes that occur. The literature will be reviewed to highlight the role that patients' growth patterns play in determining treatment outcomes and long-term stability.

The secure position of four orthodontic mini-implants in the anterior palate allows the transmission of great expansive forces via a pure bone-borne expander (MICRO4 expander). A novel force controlling and polycyclic activation protocol (FcPc), with twice a day opening and closing the hyrax screw, allows to weaken and subsequently widen the mid palatal suture in patients between 18 and 57 years up to 17mm without the need for surgical assistance. With this technique the opening force is limited, - not exceeding 500cN at the end of the activating wrench. Following this procedure for up to four months, the success rate for reaching the desired and needed maxillary width is up to 80% without surgery.

  • Understand the 4 keys to successful adult maxillary expansion: Correct anatomical position for palatal miniscrews, ideal miniscrew design, rigidity of the expander and novel force controlled polycyclic activation protocol.
  • Differentiate between presently used straight forward opening protocols and the novel way of using an expander as a force measuring instrument in combination with polycyclic opening and closing.
  • Use this knowledge to reduce your need of surgical assistance in adult maxillary expansion (80% success).

I used to feel that virtual orthodontic consults were simply a tool for quality orthodontic offices to provide potential patients with the convenient options they are requesting, and possibly engage these patients before they choose direct to consumer treatment — a marketing concept that has been working successfully in my practice for quite some time. However, an unprecedented global pandemic changed everything. We were forced to develop a complete virtual treatment platform to ensure that we could continue to manage our patients’ treatment during mandated closure of our physical office. We made the decision to be virtually open to serve our patients even though our offices were physically closed. And serendipitously, my team and I learned a lot about the potential for virtual visits to enhance (not replace) our in-office patient experience to help us deliver quality orthodontic care in an effective, efficient, and convenient manner — a mindset change that is helping us thrive in challenging times.

The mini-implant assisted rapid palatal expander (MARPE) has evolved since the early 2000's. The use of Maxillary Skeletal Expander (MSE), a particular type of MARPE, has been steadily growing globally. With a proliferation of MSE related publications by multiple authors, there is clear evidence to support its impact. This presentation will cover a brief overview of its evolution, the current usage, the latest developments and the advanced applications of MSE. Some recent discoveries will be explored with research data. Clinical cases involving non-surgical midfacial expansion in both adolescent and adult patients will be examined in detail. Secondly, other advantages of utilizing MSE for patients with nasal airway obstruction, Class III skeletal relationship and other complex problems, will be demonstrated.

Learning Objectives:

  • Understand that a different appliance design can produces different results, and that non-surgical skeletal expansion and protraction in mature patients is possible.
  • Witness MSE could be combined with less invasive surgical procedures for more challenging cases.
  • Appreciate other advantages of using MSE, particularly for patients with high angle, nasal airway obstruction, Class III skeletal relationship and other complex problems.
