When skeletal discrepancy exceeds the limits of dental compensation, orthognathic surgery may be the ideal indication. However, an increasing number of adult patients demand the use of aligners as a treatment alternative, and orthodontists and surgeons are facing difficulties to adapt to this variation. A new protocol for treatment planning, fixation technique, and clinical steps is necessary. The aim of this presentation will be to show the sequence of treatment clinical procedures to obtain optimal results. The level of evidence will be evaluated examining the available literature.
