The Maxillary Skeletal Expander (MSE), a particular type of MARPE, has evolved since early 2000, and its application has been growing globally; however, MSE’s success has not always been predictable, and enormous efforts have since taken place to improve its design and clinical protocols. Furthermore, its impact on airway/OSAS and long-term stability has been questioned and debated in recent years. The true impact of MSE for OSAS has not been clearly defined.

In this presentation, the latest updates on the appliance design and clinical protocol changes for achieving a successful outcome will be discussed, and novel minimally invasive surgical procedures designed to assist MARPE expansion will be presented. The latest findings regarding the impact of MSE on airway obstruction and OSAS will be closely examined. Recent research studies related to the stability of MSE will be presented. Lastly, the application of MSE with aligner treatment will be discussed, as this topic has been popular in recent years.
