Patients with impacted maxillary canines present with significant variations and require a team approach by an orthodontist and a periodontist. Traditional two-dimensional radiographs do not allow differentiation of the impactions beyond labial and palatal. These diagnostic limitations often resulted in compromised esthetic periodontal outcomes of the uncovered canines and prolonged treatment time. Today 3D cone-beam imaging technology offers improved diagnosis of the impacted teeth and serves as a valuable communication tool between the orthodontist and the periodontist. This presentation will introduce a comprehensive 3D Guided Classification of the maxillary canine impactions integrating the diagnosis with surgical and orthodontic management. This classification is taking into consideration risk factors that may compromise periodontal integrity of the exposed canines or adjacent teeth and anatomic landmarks critical in planning surgical exposure and orthodontic traction.
