Cuisine Art Orthodontics: Slicing and Dicing to Relieve Lower Crowding in the Mixed Dentition

Lower incisor crowding and disruption in arch integrity are early benchmarks of a significant tooth-size, arch-size discrepancy in the mixed-dentition patient. By definition, guidance of eruption procedures are designed to address such eruptive malpositioning problems through clinician-directed interventions that optimize the eruption and alignment patterns of the permanent teeth. This presentation presents a staged and sequenced protocol for such guidance of eruption interventions in dealing with lower incisor crowding in the early to mid-mixed dentition. The procedures are applicable when the patients overall arch dimensions are adequate to accommodate a normal complement of permanent teeth with acceptable esthetics and a functioning occlusion.

Learning Objectives:

• Demonstrate the application of simple and predictable guidance of eruption interventions that are effective and efficient in the early to mid-mixed dentition age child for the relief of normative levels of lower incisor crowding
• Provide a sequenced approach as to the timely use of disking of primary teeth, selected extraction of primary teeth, and placement of bilateral space maintainers in the staged supervision of leeway space

Incisor Trauma and Early Orthodontic Treatment

Incisor trauma in the developing dentition is a common occurrence that can have significant long term effects. This lecture will review the epidemiology and risk factors associated with incisor trauma. Risk factors such as increased overjet, lip incompetence and class II malocclusion have been used as justification for early orthodontic treatment in the mixed dentition to prevent future incisor trauma. This lecture will present an evidence-based review of the effectiveness and efficiency of early orthodontic treatment in preventing incisor trauma.
