Occlusion & Temporomandibular Disorders: A Paradigm Shift

Dr. Michelotti will begin by defining Temporomandibular Disorders. She will analyze the associations between malposition of jaws, occlusal factors, and TMD and the hypothesis that orthodontic treatment can prevent or cure TMD. In contradiction, the scientific evidence published over the years on the effects of traditional orthodontic treatment have firmly supported the neutral effects on the TMJ and on TMD in general. One possible explanation for the large controversies regarding the cause-effect relationship between occlusion and TMD can be found in the definition of “occlusion”. The broad concept of occlusion does not only involve the peripheral input (i.e., the tooth-to-tooth contact), but also the way the same stimulus is interpreted by the brain.
Learning Objectives:
Understand the relationship between “occlusion” and TMD according to the paradigm shift that requires moving to a more comprehensive interpretation of the concept of occlusion and of the maxillo-mandibular relationship.
Understand that patients’ occlusal awareness and concerns must be seriously accounted by clinicians during general and dental examination as a specific group of patients might develop iatrogenic maladaptive behaviors.
Understand that dental professionals and orthodontists should move away from the traditional mechanistic assessment of occlusion, and learn how to diagnose and manage TMD with conservative and reversible therapies.
