Conventional tooth-supported palatal expanders can at times produce unfavorable periodontal outcomes. The introduction of the temporary skeletal anchorage into rapid maxillary expansion appliances eliminates orthopedic forces applied directly to the teeth. In doing so, orthodontists can avoid adverse periodontal effects, achieve required skeletal maxillary expansion in non-growing patients without surgery and control skeletal and dental post-expansion movements in three dimensions.

Premolar extractions are often necessary in the situations of severe crowding with objectives to minimize periodontal attachment loss and optimize esthetics. This webinar will discuss TAD-assisted expansion protocol to allow periodontally acceptable non-extraction therapy.

Class II correction with aligner therapy is difficult, but there is a misunderstanding as to why. It is not difficult to plan treatment and hand out aligners, it is difficult to overcome the psychological barriers to patients wearing elastics when they do not want their orthodontic treatment to show. This difficulty is compounded by the inability to identify in advance which patients will wear elastics from those who will not. This leaves the orthodontist struggling with a patient who will not allow treatment to be completed without fixing the overjet, but who is unwilling to wear elastics to make the correction. Aligner therapy can be made more predictable (and enjoyable) if this dynamic is recognized, appreciated and if we can find ways to work around the trap. This lecture will explain the underpinnings of the psychology around “loss aversion” and present a creative way to correct Class II malocclusions predictably with aligner therapy.

  • Comprehend the concept of “loss aversion” and how it impacts patients motivated by dental esthetics.
  • Evaluate how loss aversion undermines the use of elastics with clear aligner therapy making Class II correction problematic and unpredictable.

Open bite represents a serious challenge in dentofacial orthopedics. This lecture will describe the dentoskeletal effects produced by different types of appliances for the treatment of dentoskeletal open bite in the mixed dentition (quad helix with crib, open bite Bionator, removable plate with crib, rapid maxillary expansion and vertical pull chin cup). The effects produced by rapid maxillary expansion in prepubertal open bite patients will be also illustrated. The long-term stability of open bite treatment will be analyzed by reporting the results of 2 studies on the effects of quad-helix/crib therapy and rapid maxillary expansion in association with posterior bite blocks in open bite growing patients.
